Membership dues are required for trips and are non-refundable. No trip application will be accepted without paid membership and proper deposit.

Complete trip applications are required at least four weeks prior to trip departure. Checks or money orders are required for deposits and payments. No cash will be accepted for trip deposits or payments.

Balance due dates are published each trip. Failure to make the balance payment by the due date may result in cancellation of the reservation. Minimum cancellation fee for ANY REASON is $25 for Midwestern trips and $50 for Western/European trips plus any travel agency fees, whatever they might be. Cancellation penalties vary by trip and every effort is made to minimize penalties. Please check with the trip leader for details. Fleetwind Ski Club encourages the purchase of trip cancellation insurance when it is available. Cancellations could result in the loss of the deposit plus total costs incurred by the club and may be as much as 100% of the trip price depending on the date of cancellation.

AGREEMENT AND RELEASE OF LIABILITY ___________________(Initial)

I am aware that snow skiing, snow racing, canoeing, whirleyball and other activities are hazardous activities and I am voluntarily participating in these activities with full knowledge of the danger involved. I hereby agree to accept any and all liability and any and all risks of injury or death. I hereby agree to accept any and all liability and any and all risks or injury or death caused during this trip by acts of terrorism, acts of declared/undeclared war, civil war, insurrection, rebellion or revolution, acts of God and/or other acts not under the control of Fleetwind Ski Club ____________________(Initial).

In consideration of the sum of One Dollar ($1.00) and other good and valuable consideration, receipt of which is hereby acknowledged, I hereby agree that I, my heirs, distributees, guardians, legal representatives and assigns will not make a claim against, sue, attach property of, or prosecute Fleetwind Ski Club and its officers, agents, representatives, trip leaders, and members for any injury or damage that may result from my participation in any snow skiing activities or any other Fleetwind Ski Club activities whatever caused by the negligence or carelessness on the part of persons aforementioned or Fleetwind Ski Club, or otherwise. In addition, I hereby release and discharge Fleetwind Ski Club and its officers, agents, representatives, trip leaders and members from any liability and any and all actions, claims or demands that I, my heirs, distributees, guardians, legal representatives, or assigns now have or may hereafter have from injury or damage resulting from my participation in any and all snow activities or other Fleetwind Ski Club activities whatever caused by the negligence or carelessness on the part of persons aforementioned or Fleetwind Ski Club or otherwise. I further release all officers, agents, representatives, trip leaders, members and Fleetwind Ski Club from any claim whatsoever on account of first aid treatment or services rendered to me during my participation in the above named trip.

I HAVE CAREFULLY READ THIS AGREEMENT AND FULLY UNDERSTAND ITS CONTENTS. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT BETWEEN MYSELF AND FLEETWIND SKI CLUB AND SIGN IT OF MY OWN FREE WILL.
